Assisted Outpatient Treatment

What is Assisted Outpatient Treatment (AOT)?

Assisted Outpatient Treatment (AOT) is a court-ordered, community-based mental health treatment program designed for individuals with severe mental illness who have a history of treatment non-adherence that has led to repeated hospitalizations, incarceration, or dangerous deterioration.

AOT allows individuals to receive structured treatment while living in the community—helping prevent crisis situations before they occur.

Why AOT Matters

Many individuals with serious mental illness experience anosognosia, a neurological condition that impairs their ability to recognize their illness. As a result, they may refuse treatment—even when they clearly need it.

Without intervention, this can lead to:

  • Repeated psychiatric hospitalizations

  • Encounters with law enforcement

  • Homelessness

  • Incarceration

  • Harm to self or others

AOT provides a proactive, compassionate solution by ensuring individuals stay connected to treatment and support.

How AOT Works

AOT is initiated through a legal process and includes:

  1. Petition to the Court
A family member, provider, or other qualified individual files a petition.

  2. Clinical Evaluation
The individual is evaluated to determine if they meet criteria.

  3. Court Order
If approved, a judge orders a treatment plan.

  4. Treatment Plan
    This may include:

    • Medication management

    • Therapy

    • Case management

    • Substance use treatment (if applicable)

  1. Ongoing Monitoring
Providers work collaboratively to support adherence and progress.

Who Qualifies for AOT?

While criteria vary by state, individuals typically qualify if they:

  • Have a serious mental illness

  • Have a history of non-adherence to treatment

  • Have experienced repeated hospitalizations or incarcerations

  • Are unlikely to survive safely in the community without supervision

  • Are at risk of deterioration or relapse

Benefits of AOT

Research and real-world outcomes show that AOT:

  • Reduces psychiatric hospitalizations

  • Decreases arrests and incarceration

  • Improves treatment adherence

  • Enhances quality of life

  • Reduces burden on emergency systems

  • Supports long-term stability and recovery

AOT shifts care from crisis response to early intervention.

AOT vs. Inpatient Commitment

 AOT                                                                        Inpatient Commitment

Community-based                                               Hospital-based

Preventative                                                          Crisis-driven

Maintains independence                                    Restricts freedom

Long-term support                                              Short-term stabilization

AOT is designed to prevent the need for hospitalization, not replace it.

A Compassionate Approach to Care

AOT is not about punishment or control—it is about access to care.

It provides a structured pathway for individuals who, due to their illness, cannot consistently make decisions in their own best interest. By intervening earlier, AOT helps preserve dignity, safety, and long-term outcomes.

AOT in Practice

States that have implemented AOT programs—such as New York’s “Kendra’s Law”—have demonstrated significant success, including:

  • Up to 77% reduction in hospitalizations

  • Significant reductions in homelessness and incarceration

  • Increased engagement in treatment

Why We Advocate for AOT

We believe that no family should have to wait for a crisis to access care.

AOT provides a life-saving bridge between independence and intervention—ensuring individuals receive the treatment they need before tragedy occurs.
